Ready to learn more about weight loss surgery options?  Here are 4 steps to keep in mind as you go through our online seminar.    You may also want to have a pen and paper handy to write down any questions you may have for our team.

  • Sign on to the seminar by completing all fields.  This will automatically trigger an email to your inbox.  Be sure to save this email for future reference: it includes your personal link to the online seminar so that you can resume watching if you have to close out before you finish the full session.  Check your spam box if you don’t see the email message in your inbox . If you misplace this email, you can always login here .
  • Watch the seminar in full. It lasts about 40 minutes. If you step away from the session, you will be able to resume the seminar where you left off on your device. If you close out of the session before completing it, you can take the seminar again by clicking on the personal “Rewatch Seminar” ink in the email you received from us.
  • When you are done with the online seminar, if you are interested in pursuing weight loss surgery, you will be able to download or print the New Patient Form at the end of the seminar. Your pop up blocker must be disabled for the form to pop up on screen. You can also download the form by clicking on New Patient Form.
  • After filling out the form,  please email it to mnsinfo@tenethealth or fax it to 314-984-2729. Your information goes to our insurance verifier who will check your benefits and any insurance requirements. Our verifier will get back to you via email within (10 business) days.
